Many students in our cohort have experienced inconsistent treatment from UCT. Over the past six months I have repeatedly raised these issues on behalf of the Class of 2024, yet the university has not meaningfully engaged. Completing this brief form helps us collectively insist on transparency and accountability.

This form automatically generates FORM 2 (Request for Access to Record) with all your information pre-filled. After downloading the PDF, please email it to registrar@uct.ac.za along with a copy of your ID document to request your MBChB award calculation records under the Promotion of Access to Information Act (PAIA).
